Vendor Payment Form

As you are aware, Governor Hogan has restricted large gatherings until further notice. While we are hopeful these restrictions will ease over the next few weeks, we do not know if this will allow us to hold our June Celebration. Due to the uncertainty, we have decided to cancel our event for 2020. We have already scheduled our Celebration in the Park-June 5,2021. We are providing vendors and sponsors options for how you want us to apply your payment.

Option 1-we can apply your paid amount to a vendor spot/sponsorship to our event next year-this will secure your vendor spot/sponsorship.

Option 2- The Smitheburg Days Committee has decided since we can not hold our annual event, we will be making a donation to Tabitha’s Table. You may elect to include your paid fee/sponsorship in this donation and we will include you on our announcement as a “Thank You”.

Option 3- We can issue you a refund. Refunds will be issued in the next 60 days as we work through the list of requests.
